Questionnaire – Part 1

Thank you for completing the questionnaire.

Data of all the educators under your service provider will be grouped together and fed back to your service provider. The data will be used for the Quality Improvement Plan. The next step is to submit a self-assessment reflection. You may choose to reflect on the results from your questionnaire.

You will receive a score out of 3 for each question based on your answer, with 0 being the lowest score and 3 being the highest score. There are suggested resources for each question that you can choose to view and download, should you wish to use these resources to address your priority areas.
